How to leave feedback on purchased item (purchased 4 mo ago)?

How do I leave feedback on a product that I purchased 4 mos ago. I went to my ordered item and there's no option to do so. TYMnSue

I have returned an item and told I would receive my refund when the item is delivered.

The tracking number shows that the item was delivered and ready for pickup at a post office. I still have not received a refund. What happens if the seller never picks up the item since it is so badly damaged it is of no value to anyone.
