I just sold an item. The item description states No International Shipping, yet the buyer's profile is from another country. I still don't want to ship internationally. What should I do?

If the shipping address is a USA address, ship it. Don't question it, don't NOT ship it unless you want to have your account permanently banned from selling.
You are NOT shipping internationally, so the purchase is NOT going against the way you listed it.
Many 'international' buyers use a 'freight forwarding' service to collect many items and have them shipped all together for a much lower cost.

What is provided for a "shipping address"?
Some buyers use a "freight forwarder" and you ship to a USA address.

I don't understand how you can ask this question.
Does it not tell you the shipping address on the order?
Is it to the US or not ?
You seem to be under the misconception that no international shipping means no international buyers. That isn't true.

You may be hit with the international fee (1.65%), that's about it, even though the shipping address will be in the US.
