Hi, I recently sold a The Art of Shaving Engraved 100% Silvertip Badger Hair Shaving Brush NEW with a photos. The buyer got the item today and requested a return.

Sorry, I have this set, razor, stand and brush. This brush I have purchased from you does match the razor handle. What does not match is the engraved diamond pattern. The diamond pattern on the brush is noticeably smaller than my current brush and it’s matching razor. If it wasn’t so obvious I would not mind. But the difference is distinct. I would like to return for refund.
On the return request page, it says at the top "The buyer requested a return, but you're not obligated to accept a return for this item.
Paypal already put a hold on the money

I don't believe you are required to accept a return of a custom made item. If you decline the request PayPal will eventually have to release the funds or return them to the buyer. However, I would check PayPal's policies regarding custom made orders.
Of course, declining the return will probably get you a neg but it should, in my opinion, be removable.

You don't have to accept the return ... but, if you do not respond to the return request (by denying it), then eBay will auto-accept it for you.
Sooooo ... make a decision.

If looks like your listings have no returns since the buyer filed a remorse reason you can decline the return
