In the past 2-3 months I have had to open about 5 return requests on items because of shipping damage, items not as described, and wrong items being sent to me. EBay gave me a warning today saying I opened a lot of returns. Should I be concerned?

Well keep it up and you will lose your buyer protections. eBay thinks the percentage of bad transactions you have is above average.

Looking at the feedback you've left makes me think you need to vet your sellers better.

Unless they sent you some other message, I don't see that as a warning. But I have to wonder ... 5 out of how many total transactions?

Yes, you do need to be concerned. eBay may void your buyer protection because they think you are abusing it, and you will no longer have any eBay buyer protection if that happens.
More advice, you are buying and selling on the same account, and that can mean trouble. What will you do if an angry seller comes back as a buyer to leave negs or neutrals for you? Use a different account for buying.

I am glad that ebay has a system in place for people that abuse the return system, the problem is I mostly buy from small sellers that dont package items very well and they get damaged in shipping. I dont think anyone would be happy with a damaged item and that is why I have this warning.

5 out of 20 is VERY EXCESSIVE.
I had to really think hard to remember my last return. It was either a parrot feeder that arrived broken (Sid my cockatoo passed away 3 years ago) or a waterproof cell phone case that wouldn't stay on the phone, (several phones and years ago).
I primarily buy fragile items, LP records. I have had some larger lots arrive looking kinda rough but thankfully nothing damaged.
My guess is that you either are not Veting sellers AT ALL or in the habit of phishing for partial refunds.

This is definitely a warning from eBay, they are letting you know that you are risking your ability to make any return under the eBay MBG. I have purchased hundreds of items on eBay over the past 14 years and have had one broken item (shattered glass in a picture frame) and had one wrong item sent (intentionally due to out of stock) and 95% or more of my purchases were from small sellers. You have left many "wrong item sent, seller refunded me or item arrived broken or damaged" fb for sellers. These sellers may be reporting you also, which you would not know but eBay keeps tabs on.
