Can't combine shipping on multiple "Buy it Now" items

Even though the box that says "Require immediate payment with Buy It Now" is unchecked in all my items, multiple customers have reported that when they try to buy multiple items, ebay reports that I do not combine shipping and will not allow them to purchase without immediate payment.

Is there a setting I'm missing.  It's worked properly in the past, but not now.

I just added two items to my cart and it let me select "Request Total".  The buyers need to add the items to their cart in order to request combined shipping rates.

Did you actually click on request total?  That would be when the buyers would see that the seller doesn't have combined shipment.

I clicked on Request Total and it showed me the screen listing the items that would be sent. I did not proceed as I did not want to actually agree to purchase the items.

Do you have combined payments and shipping allowed in your shipping preferences?

If one of the items in a cart is on sale or has free shipping or has seller initiated ipr, the request total  button in the cart won't work. I was under the impression though that it would just be greyed out in those cases so in those cases, the buyers wouldn't be able to click on it at all or receive a seller does not combine message.
