When I provide a buyer 100% refund, then I don't think is fair to allow the buyer to further harm you with negative feedback.
Regarding returns, this automatic 30-day return policy if it doesn't match is a license for the buyer to stela from you, which happens a lot now a days.
Two more reasons why as a business seller I am leaving eBay soon!

Even with a refund, the buyer is allowed to express their feelings about a bad transaction.

Just because the buyer got a refund does mean the buyer is happy with the transaction. What if whatever he bought was something he really needed, or really wanted? What if the product was so defective its use caused damge to something else? What if he didn't find the refund process as easy as it might have been? What if it was a gift and it turned out to be too shabby to give to someone? What if he resented the time he lost on this whole thing? In short, getting a refund doesn't always a satisfied buyer make.

Did the buyer ask you to cancel and refund their money? I don't know the details of why they didn't get their item they paid for but I'm sure they're disappointed they wasted their time and money.
