If I set my listings to where I don't accept returns, and a buyer wants to return an item.
If they choose INAD, is the return accepted automatically?
Or do I accept the return on my end? (I know I must for INAD).
Are there any other reasons that a buyer can choose where I must accept the return?

My question remains: What other reasons MUST I accept the return?
1: Doesn't work or defective
2: Doesn't match description or photos
3: Wrong item sent
4: Missing parts or pieces
5: Arrived damaged
6: Doesn't seem authentic

can't answer that, but what I have done is set my policy to "add an RMA #" which stops the "automatic return" allowing me to choose what action I take/when I take action.

All return types are 'automated' unless you 'require an RMA number' (that's what I do so I can
a.) add an RMA for no reason as I don't get enough returns to need it
b.) it 'stops' the 'automated' so I can see it before it's approved, sometimes I contact the buyer, find out they don't need to return it, decide to keep it etc. and then I can 'accept' it, when it doesn't ship back in 3 weeks, it goes away.

I have a no returns policy also and I've never had any return request automatically accepted. How would that even work? EBay would just send a return shipping label at my expense just because someone opens a return request? I also have never accepted a return because I've never had a return request on anything that had enough value to cover all the shipping cost so I just refunded.
I think you would need to offer free returns for eBay to automatically accept returns.

It's been a few years since I had a return and I accept returns.
I've had some INAD's auto accepted and some I had to accept.
I don't know what the difference was?

EBAY return system is "automated" to accept for BOTH "not as described or does not fit" even if you have a "no returns" policy. Of course, it reason for return is "buyer's remorse" you do not have to accept it if you have "no returns" policy.

I have a "no returns" policy, and have received a few remorse/doesn't fit return requests over the past year.
I have accepted all of them manually; nothing was auto-accepted.
I believe it's because I have RMA Number enabled.

That can't be right.
I accidentally submitted a whole vehicle (well, 200 parts off of one vehicle). And somehow my settings were "no returns" on this lot of parts.
When somebody wants to return an item off of that vehicle. I see "return requested" rather than "return approved".
Seen three return requests. I denied two of them (changed mind). And I accepted the INAD request because....I basically KNOW that I must.
My question remains: What other reasons MUST I accept the return?

And see, there's a difference there as well. If they buyer follows their protocols for Free Returns when buying..AND...you have Compatibility Settings filled out. Then a Doesn't Fit means the seller pays the return shipping.
IF the above is not followed by the buyer AND the seller. Then a Doesn't Fit return cost is payed for by the buyer.
