Simplest thing to do is tell customers you will refund what is NOT used.
After all, 1 Dress may cost $8.99, but 2 dresses require a bigger package and more weight and may cost $12 to ship....

From the shipping label page you can click on different orders (as long as it's the same buyer) and combine multiple orders in to one shipping label.

That is one way. But I wouldn't handle it that way because there are costs beyond the literal cost of the shipping to be considered IMHO. There are fees as well as shipping supplies.

There are different ways to do this. I do not trust eBay's automated system, so I used an antiquated method that has served me well for many years.
1. Look at the multiple orders and determine what is your combined shipping cost.
2. Ask the buyer to purchase all the items at the prices indicated. Tell the buyer that you will refund them the difference between the shipping they purchased and the combined shipping cost that you charge.
If the buyer is reluctant to do this (because of worry that you will NOT refund them), you can offer to create for them a new listing that groups all the items they want, with a single shipping charge.
You can also explain that you would never renege on your word because eBay has access to all message traffic and you, as seller, would be penalized if you did. (Not to mention the buyer would almost certainly leave you negative feedback.)

For me, if a buyer buys multiple items & doesn't ask about combining shipping then they get multiple packages because that's what they paid for. I'm not messing with combining orders & partial refunds

This is the way that I do it, I used to set up a special listing but I quit doing that because 1/2 the time after doing it you never hear from them again.
I remember one time I had like 10 cases of heavy light ballasts and someone asked if I would combine shipping on all of them and I told them I would combine them, ship them UPS instead of priority and refund the overpayment. I had originally had them shipping in regional b boxes and the combined UPS shipping was higher than the priority, I could not charge any more for shipping and the buyer did not get a combined shipping break, neither one of us were very happy.

Hmmm I notice not many sellers do it from the shipping label page like I do. I wonder why? That's what it's there for.
