I don't accept returns. His reason is, 'Changed my mind." And under comments he wrote, "Attempted to cancel it before it shipped but eBay wouldn’t let me. I never even opened it." It was sold with free shipping which cost me $4. If I decline, can he leave negative feedback? If so, can I get it removed with the reasons and comments he said?

Yes. They can leave a neg no matter what you do. They can leave it now if they like. It is probably not a good idea to conduct business in fear of a red donut. They don't count for anything anymore, and you could always provide a professional sounding response.
How much are you willing to pay just for the "chance" of not getting one?
Many many many years ago you could pay a "third party" $25 to have one removed. I always wondered what was eBay's share.

With respect, this reply right here indicates you don't seem quite cut out for online selling. You are selling to perfect strangers across the country. There is an inherent risk to anything sold on this platform, which caters to the buyer and not the seller. Being fearful of a negative comment, especially when you haven't even received one yet, indicates you are focusing on the small details. Feedback is one person's opinion and difficult to get removed. Just ask buyer to return the item and send him the refund. Done.
