I made my first sale off a pair of women's boots. It my listing I made it clear there was no size marked but that it fit a women's size 7/7.5 based on me trying them on (I am a true size 7). I shipped to the buyer and they claimed the sizing was off and they couldn't fit their 6.5 foot into the boot. I didn't know how to dispute it so I paid for the shipping label and had them send the boots back, feeling bad like maybe my sizing was off. The boots arrived today, and as suspected they do fit a true size 7. Do I have any option here, or should I just issue the refund?

Nope

Just issue the refund: and report the seller for returns abuse.

You must refund; or ebay will do it for you when the buyer "escalates" and the return is shown 'delivered' back to the seller.
Size is your word against the buyers, so 'reporting' does nothing and there is really nothing to report.

U.S. women's shoe size 7 is size 4.5 in U.K., 5.5 in Australia, 23 in Japan, 37.5 in Europe, 4 in Mexico, etc.

In the future you might consider adding some measurements to the listing. Also, if you had a return policy, then buyers will be more likely to return due to fit and be responsible for the shipping. Otherwise, if they file a not as described return, you have no good options other than issue the label and refund when they are returned.

Issue a refund and re-sell the boots.

If you issue the refund, you avoid the chance of a very damaging defect.
If you don't refund, the buyer will escalate, eBay will force the refund anyway, and you will get a very damaging defect.

Sorry OP but you must refund. A buyer can return for any reason basically and the seller must refund.
And not to sound glib but be lucky you received back the thing you sent, they could've returned to you a dirty sock and you'd still have to refund. 😞
Better luck next time!

Additionally, sizing is very finicky with shoes/clothing. A person might think they know their size, or in fact know their size with one brand of clothing/shes/whatever, but then another brand has slightly different sizing.
Even buying clothes/shoes from retailers is tricky and often generates returns. I can only imagine it would be even more frequent with people selling pre-owned items.

I don't think you were duped. I think the boots didn't fit. Maybe a problem with width, or with ankle or calf size. Boots are harder to fit than shoes. The fact that the buyer returned them to you promptly and in their original condition makes me think there was a problem with the fit.
In any event, you must issue the refund, whether or not the buyer was being honest. And I'm sorry your first sale did not go smoothly.

1. Of course, you have to issue the refund.
2. Maybe, before listing them again, you need to measure the sole carefully and be sure to explain that there is no marked size.

These boots were made for walkin' and that's just what they'll do.
One of these days these boots are gonna walk all over you. 😃

The buyer paid for the boots and you got your boots back. You should refund your buyer.

@amamcca_41 Don't delay in settling this, either - ebay doesn't give we sellers much of a time frame for resolution and you don't want that black mark for an 'unresolved case'.
