We have accepted two offers and now they have just disappeared. Anyone else seeing this?

Were they BUYER generated offers? i.e. the buyer sent them to you and you accepted (not counter offers)?
If they were, your settings require that the buyer put up a payment source in order to make the offer. As a result, when you accept the offer, the item is immediately billed and paid for. Invoicing or combined shipping for the same buyer is not available.
If you SENT an offer, and accepted a counter offer, the buyer is not informed that you did so. If they happen upon the right page, otherwise they likely don't even know.
All offers you send will be immediate payment required (including any counter offers you accept). Invoicing/combining shipping will not be available.

I'm guessing the most likely scenario here is they can be found when you review offers for those listings and check under Pending.
This is not an error, it's something eBay has been testing with accepted offers - they remain in pending status while the item is available for anybody else to purchase until the buyer pays. It's one of several things eBay is testing to cut down on non-payers and the inconvenience to sellers when buyers lose interest after an item ends, and seller has to start from scratch when they relist. If the buyer does not pay within a certain amount of time it will time out and be removed from the pending section. If the buyer pays then it will become an official transaction on your orders page.
