....three months later.
"Hello, good evening, I just checked my package and it is not what I expected because I was expecting the carabiner in the main photo, not the small one."
The caveat being that the reference to "the small one" is that there was an error with the photos. Not a big one; there was one incorrect photo showing a slightly different model of carabiner than what was sent, but an error all the same. I have to own that, even though the correct item was sent. Had this been within the thirty day return window, I wouldn't have hesitated.
It's because of this that I am torn.
Asking for a return three months later is no bueno for a myriad of reasons.
....but then of my listing fumble. I want to be honest and fair, but three months later? Does that negate that three months of nothing?
What would you do?

It really depends on how you do business.
I would take the item back, and I have a no returns policy.
If it was my mistake, the buyer's bad judgement does not change that. And I behave based on what I think is right, not what anyone's rules allow me to ignore.
But I understand why you might not want to take the return if not forced to.

Reverse positions, and have a look see.
I would likely do the same thing at 90 days that I would do at 2 days.

Thanks for the reply, and point taken. That IS how I do business, but the three months gave me pause. Appreciate the input.

I would ask "why wait 3 months'?? Problem is you are not in my system after 60 days to issue a refund once you return it".....
Too late for them to 'open' a return and too late for you to get 'returned final value fees' so either refund minus fees (if you want; after they return it or?)
