I did and he has til 6/4 to issue refund but he has contacted eBay stating I sent him a fake item back. What happens in that situation? He literally lives in a camper by a lake when you search the return address. He is obviously a professional scammer.

You need authenticated proof the item received was a fake. Just your word might not do it.
It's he said/she said, and you may need to tip the scales more in your favor.

eBay may just grant a courtesy refund.

Where the seller lives has nothing to do with anything. And you do not know that "obviously he is a professional scammer".
If he has until 6/4 to issue a refund and does not do it by that date, ask eBay to step in and they will force the refund for him. If he has a record of doing this, they will make note of it. If he does not have a record of doing this, they may pay the refund themselves and not charge him this one time, so in other words, both of you will be made whole. Please note: I do not know that eBay will handle it this way; I have only read about it happening sometimes in a "he said - he said" issue.

I do have some pictures and it was an obvious fake Appel watch put in a real Apple Watch box. Charging cable fake with watch that wouldn’t power on

He had no negative reviews that is why I purchased it but once I received it I realized it was a fake Apple watch ultra placed in a real Apple Watch box.

I did but when he received the return he claims that I sent a fake item back when he was the one who sold me a fake item.

Many sellers have received incorrect items back in return cases and nothing can be done about that, they have been scammed by the buyer. It’s a risk that sellers take on eBay. Similar to you as a buyer that received a wrong item. It’s a he said/she said situation. eBay has no idea what was originally sent or received in return. There is no proof. Just wait till 06/04, and if no action, ask eBay to step in.

Looks like the seller is dropshipping, so they have no idea what their supplier is shipping out.
Seller might not know a fake was shipped out, but a fake was returned.

Is it possible to do 'drop shipping' if items sold and for sale state 'for local pick-up'?
Wouldn't the person who sold it have to be there to give item to buyer.

Yes, there's some warehouses where you can go pick up the items.
Even if they dropship from a retail store, the order would be under the buyers name so they could pick it up.
