After selling two items, where I took offers, I was met with extra shipping fees that I was not expecting. Today, I realized over half of my listings were somehow changed from my standard calculated shipping to a flat rate shipping no matter where the buyer is located. This was without my knowledge or consent. How does something like this happen? I have manually reviewed and corrected all my listings.

Not sure. Opt into business policies if you're worried about it happening again and dread the thought of having to spend all that time fixing it. Manual review & correction of a handful of shipping policies would be orders of magnitude faster than doing the same for even a quarter of your listings.
You have free shipping on a not-small cast iron item. Just pointing that out in case it wasn't deliberate.

Don't know about all that; but next you'll be posting about items being returned because they are damaged and buyer says 'not as described' and YOU are paying the return shipping....all because you have items listed as 'not returns' and what you really need to know is:
'no returns' does NOT mean 'no refunds'.
