Scan Forms Not Generating

Been using bulk shipping and generating scan forms for years but a couple of weeks ago I started getting this error message: 

Bulk label printing error

Error generating scan form

I have reported this to customer service several times, they said they would forward to tech but I have not heard anything.  US Postal Service is not happy to have to scan every package now.

Any eBay techs have an answer?  I have changed browsers, cleared cache, deleted history and done everything I can think of to get the scan forms to generate again.  It has been 3 months since the "generate scan form" has been giving me the same sad faced box error. 

The only thing that I can think of that changed was that I changed the "shipping from" address to reflect the correct zip code as requested by the Post Office that I use.
