Leaving feedback on app

Just went to leave feedback using my android phone and one of the options just says "yes" or "no". I think this is normally supposed to have something similar do "did this arrive by xx/xx/2019". I attached a picture

I usually leave feedback with a browser, but I just tried with the app and I am seeing the exact same thing. I tried scrolling up and down to see if the text would be revealed, but had no luck with that. 

I was still able to successfully leave feedback otherwise, though.
