If an item arrives late and a refund was already issued, can ebay reverse the refund or does the buyer get both the item and the money?

Right now, the buyer ends up with both.

All you can do is ask the buyer if they'll repay.

they get to keep both, ebay cannot "take the money back"

if they do not pay you out of the goodness of there heart, then just chalk it up as a tax write off at the end of the year.

it happens..... thankfully pretty rarely though

usually if the package is running late you can request extra time with customer service, if the tracking shows it was shipped on time and is moving thru the system, but normally that only gives you a few extra days.
