The "Top Rated" tag is based on the number of sales a seller has made. It is not based on a sellers performance even though you may see "consistently delivers good service".
When you open a listing page, if you scroll to the bottom of it you will see the seller's name in a large font, then a couple of feedback comments. Below that is an oval with See all feedback written in it. Click on that button and on the page that opens you will see a box with the 3 types of feedback a seller can receive, you can click on the numbers of neutral and negative feedback to read only those comments.
The rule of thumb when buying from sellers with a high number of feedback is not to buy from sellers whose feedback percentage is below 98.6%. My personal cut off point is 99.0%. Be very careful of buying from high feedback number chinese sellers. Never go by a feedback percentage alone.

Have to check their feedback rating. Also, always pay by credit card for the added protection they offer.

As mudshark states, it's based on their sales number. "Top rated" doesn't necessarily make them a great or honest seller.
