If I enter a dollar amount for shipping an item does the buyer pay that including the purchase amount

Yes, the buyer pays the shipping charges as shown on the listing page. They pay (through Ebay) for the price of the item plus your shipping charges.
To make sure you set your shipping charges correctly, you need to use calculated shipping:
Here's how to use calculated shipping when listing your items:
- In the Shipping section of your listing, select Calculated: Cost varies by buyer location.
- Next to Services, select Calculate Shipping.
- In Package details in the Shipping Calculator, enter your package type, dimensions, and weight. If you're selling an irregularly shaped item, estimate its dimensions by measuring a box you can fit it in.
- In the Your details section, enter your ZIP code and any handling fees.
- You can add a destination if you'd like. If you're shipping internationally, we recommend using eBay International Shipping.
- Select Calculate shipping.
- Select the shipping services you want to offer, and drag them into the order you want buyers to see them.
- Select Offer shipping services.
- Complete your listing.
