has anyone accepted a return by mistake. Sent item in perfect working condition, and buyer 2 weeks later saying it is defective and now return will be worthless . Anyway to fight a return

No. When a buyer says not as described, the return is automatic. You have no choice. You can issue a paid return label and refund when you get it back. There is a small chance the seller won't get around to sending it back. Or you can just refund and let them keep it for free.

You can fight it, the buyer will still get a refund from your account and they'll probably get to keep the item.
Since the buyer filed an INAD, you either accept the return or just refund the buyer without returning the item.

I listed an item using a plural noun, but specified 1 item per pack. A buyer says the plural noun implied 5 or 6 items, and wanted me to send her 5 or 6 more items for free or want full refund, not even returning the quality item I sent. Is that buyer just trying to get the item for free by making a problem?

It appears to me you're giving them several to choose from because you say choose 1 or more, and show 6 different ones to choose from, and you have prices for 1, 2, 3, etc.
She doesn't get a refund and get to keep the item. She needs to return what she received if she wants a refund. She does not get more product sent.
