I've never had this problem before, but an item I recently sold is claimed as not working. It was packaged perfectly and worked before being sent.
The eBay policy seems to indicate that I must accept the return and pay for return shipping? This is a large and heavy item so it's going to be very expensive for me.
Also, after I receive the item back and if I find that it works fine do I then have any additional options to recover the money?

As ed said, return for refund. If when you receive it back it's does function, report the buyer for abusing the MBG and call eBay. You should be able to get at least some of your shipping cost back based on buyer abuse.

If they filed an INAD, you have two choices.
Refund buyer and let them keep the item
Accept the return, supply return shipping and refund buyer once you get the item back.
If the item does work when you get it back, you can report the buyer for abusing the MBG and you can appeal and try to get your return shipping cost back.
