USPS Insurance request form

I'm trying to fill out the USPS  on-line request form for an insurance payment re a card the USPS lost.  How do I go about uploading proof of value onto the form?  Help would be greatly appreciated!!

The USPS policy says that they want documentation of your PayPal transaction, so save that page as a pdf.

PRIORITY MAIL has insurance included in postage price.  Attached copy of EBAY invoice (shows purchase price & shipping costs).

USPS is good about paying claims.  USPS will mail you a check in 14 days.
