We have several listings in the "Non-Sports Cards" > "Trading Card Singles" category (as appropriate for the items) where we ask @$2.50 to ship and .05 for each additional card. The rule is setup correctly on my listing page as well as the actual listing itself, but the system simply won't combine the shipping on the buyer's end in the cart.
I get messages routinely from several of these card listings asking if we combine shipping, and I can navigate most buyers through it by asking them to request an invoice where I combine the shipping for them, but this problem routinely causes me to lose the "nervous" or "maladaptive buyer". I really don't want to lose sales because of "functionality', regardless of buyer savvy or competency.
Anyone know a fix or aware of the issue?

Edited: May not be anything about your listings. It could be because the buyers are using the app.

@wave1collectibles It seems to only work if you select multiple quantities of the exact same item/variation. You say that you have it set up correctly, but it is worth checking as there are two settings you need to worry about for combined shipping ... one for identical items (which you have set up) and the other is for non-identical items.
Not sure if you use business policies, but even if you don't, you should find something similar. In my example, I charge 50 cents for each additional, but you get the idea. The box under "each additional" covers multiple quantities of the same exact variation and you apparently have yours set up. Now check the combined shipping discount at the bottom. It needs to be check to cover the situation where the multiple are across items or variations. Make sure you have a shipping profile set up for your extra five cents and also enabled.
P.S. When I look at your listings, it appears you may have the combined shipping discount set to $1.50 (see the green below) instead of $0.05

It does look as if you have a combined shipping rule set up for $1.50 for each additional item and you have it attached to at least some of your single card listings so that is what is being charged. I don't use business policies and will describe how I would set it up. Make a new flat rate shipping rule on your shipping preferences page for .05 for each additional card. Name the rule and save it. You should then be able to go to bulk edit and then on the bulk screen you want to go to combined shipping and change it by deleting the $1.50 rule on those listings and adding the .05 rule. Does that make sense?
If you are using business policies you probably have to make a shipping policy just for those cards.

I want to thank everyone for their input. I went ahead and made adjustments to all of my card listings and a few others as well. The shipping rule system makes a lot more sense now, although it's functionality is a bit overcomplicated and obtuse. So basically you have to go into the "Shipping Preferences" screen and set rules with names (i.e., "Cards" set at .05 cents for add-on) and then go into the listing and assign the rule for enforcement with every listing.
I think I have it setup properly now so thank you all again, although now I have another question - these rules all appear to be driven by "type" according to the rule. How do I set EVERYTHING in my store for some kind of combined shipping (i.e., "request an invoice" or "request a total") should someone want to buy say 4 cards, an order of bags and an action figure?

I'm not sure that I understand the question but there is no way to set up a listing so that a send invoice is requested. You can set up flat rate different policies. Or if you are using calculated shipping on some listings you could set up a calculated shipping rule. Just choose the applicable on one the listing.
You can also set up a promotional shipping rule on the shipping preferences page.
