I received notification on Nov. 23 from my buyer that an item she bought on Oct. 11 has been returned, which is past the 30 days. She is paying for the return shipping. Should I refund the buyer when it arrives or what do I do?

Technically, the buyer is supposed to contact you regarding a return. However, if item arrives intact, the way you sent it, I'd go ahead and refund the buyer's full payment. That way, you'll get the FVF credited.
My ebay > SOLD > More actions > Cancel order - seller selects a reason
The buyer's Paypal payment will be refunded (if buyer paid) and seller will be credited the Final Value Fee charged on the sale. ebay will send an email to the buyer to confirm the refund and cancellation. If the buyer does not confirm after 10 days, seller can close it.

You don't do anything unless you get the item back.
You can offer to return it at the buyer's expense or nothing at all.
Buyer could still intiate a paypal return...doubtful though if they don't know how to do an ebay one and they could still leave feedback up to 60 days after the sale....but that is doubtful too seeing as they don't know how to use ebay in the first place.
