Request Returns or Report wrong item received options not working

Each time I try to request a return (Received wrong item) it crashed right at the end when I click submit.

4 times I've tried including uploading photos but same outcome each time.

Browser: Opera (no I wont use chrome or firefox)

Did you try sending the request without pictures?  I don't know if that is the problem but if you haven't tried that you might want to do so.
