Filing UPS claim for item shipped through ebay

I'm a seller. Somthing I shipped with a mile of bubblewrap got bashed in by UPS and broken; I now want to file a claim.

I went to the UPS site and submitted a claim. (https://www.ups.com/us/en/support/file-a-claim.page). My UPS account and my ebay account both use the same email.

I agreed and ticked the box where it asks if both the item and box were available for inspection.

Yet when I submitted the claim, I received the message "We cannot submit your claim at this time. Please make sure your information is accurate. Otherwise, contact your Channel Partner. [UPS:E040]"

I have attempted on two other occasions to submit a claim with UPS, and the same thing happened so I eventually gave up.

This is getting too expensive. I'm not giving up this time.

I googled the UPS error message to see what I could find out. All it suggested was that my ebay email and my UPS needed to be the same, or I would get an error. It IS the same, yet I still receive the error.

Any suggestions? Thank you.

Re: Filing UPS claim for item shipped through ebay

Filing a UPS claim is different if you shipped through ebay and did not use your own account.  One more reason not to use ebay UPS, but your own account or a free PirateShip account.

If your parcel is lost or missing:

Download and and fill out the Loss Claim Form (see link at bottom).
Email the completed Loss Claim Form to claiminformation@ups.com with the subject line "Customized eBay Claims Support".

Once you submit your claim, an investigation will be conducted by UPS to try to locate the package. If the investigation is unsuccessful, you'll receive notification with next steps to file for payment.

If your parcel is damaged:

Download and fill out the Damage Claim Form (see link at bottom).
Email the completed Damage Claim Form to ebayclaiminformation@ups.com with the subject line "Customized eBay Claims Support".

Once you submit your claim, a damage inspection will be conducted by UPS. If the result of the inspection determines the carrier is at fault, you'll receive notification with next steps to file for payment.

For all damage claims, the damaged item, packaging, and all contents must be retained until the claim is resolved.

This is a handy step-by-step tutorial with images for both lost and damaged claims:
