I'm curious if anyone knows whether freight forwarders routinely open packages and repackage before shipping them to their next destination. I wouldn't care if they repackage as carefully as I do, but I'd be really unhappy if their repackaging caused damage to my item.

Yes they do. They have to make sure it is legal and safe item to travel.

Some repack in just a padded bag to save shipping costs. They also lose and mix up items when repacking too.

Yes, they do. How careful they are about repackaging depends entirely on the reshipper. The Japanese reshippers I use to purchase goods from Japanese webstores, for instance, pack extremely well. I've never had anything get damaged.
