Customs form - What changed?

I have shipped overseas many times and every time I have used eBay to print postage and also my customs form.  

Tonight, I have a package going to Puerto Rico and it asked me everything, filled it in, but it did not print the customs declaration.  Any suggestions on how to obtain it?

Shipping to Puerto Rico or any other Us territory via USPS is Domestic, not International. The only small difference between sending packages to the continental US and the US territories is the cost.  

There are no customs forms needed for Puerto Rico.
