So I sold two similar items, which were paid for promptly to someone using 2 separate nicks.
Only when I compared the buyers real name & address did I realize it was one and the same person. I sent a message asking if it's ok to combine the shipping, and I offered a refund to adjust the shipping. I finally heard back from her, she says go ahead. Well, I printed one label & tried to add the tracking number to the other item listing, but got a message in RED: You have already used that number for a different customer. Hmmm. I'm just taking a chance & shipping anyhow, although she could claim she never received the item since there is no tracking number for it. It's a low-cost item, so I doubt it's a scam, but has anyone had experience with this before?

Yes, I have experienced that. As the message states: you can't use the same tracking number for purchases by two accounts. I agree with your intuition, it's probably not a scam, especially since it was you and not the buyer who initiated the combined-shipping idea.

I am just slightly worried about how it affects my status as Top Rated Seller. There's that requirement " tracking uploaded on time and validated." Will I get a Frowny Face from Ebay now?

Were the usernames two random strings of numbers and letters with zero feedback?
If so, those are guest purchases and the buyer had no control over it but it’s not a scam. Did they pay shipping on each item? If so, you should ship both purchases separately.

Each one had a cute little username, and both had a regular name & the same address. Each had decent amount of feedback. I didn't think it was any of my business to ask WHY she had 2 accounts. Now I'm just wondering if I get a bad mark on my seller's record due to no tracking uploaded.

Personally I would ship them seperately, advise the buyer that you are doing this as eBay has no procedure in place to combine for 2 different buyer account purchases.
