If I specify a certain amount for postage and select combined shipping will this work? Or, does combined shipping postage only work with calculated shipping? I have a person that purchased 3 items and would like to offer them a discount for their postage. They have already paid the postage.
Also, how would I refund excess postage to them.

For a quantity of items in a single listing, you can show a shipping amount for the first item, and a shipping amount for each additional item. Otherwise, need more details of what you'd like to do.
For the current buyer, you can use the Refund button in the drop down menu next to the item.

A person bought 3 small individual items that could fit in one envelope for one price. I usually don't have people buy multi items. Each item had a shipping price of $3.75, which I could ship all 3 for that price. I would like to be able to offer shipping for the one price of $3.75. I would also like to refund over $6.00 in shipping that the person did not have to pay. My items are small and usually I can ship a few for around $3.50. Just don't know if there is a way to save the person buying extra postage.

Use the Refund button. You can do this on more than one listing if needed to refund the total you want.
If these are items from different listings, then you need to either combine then manually before buyer pays or set up and apply a shipping rule to do it automatically.
First you need to enable Combined Payments in your Shipping Preferences. Then buyer can add all their items to their shopping cart and click on Request Total for you to manually adjust the shipping.
Otherwise, you need to set up a shipping rule and enable it on each listing it applies to. This is also there on your Shipping Preferences page.
