Adding an item to International shipping.

I am in the US and have a buyer in Australia who has already purchased an item and paid for the GSP for it.  He wants to add another item I told him about but have not listed yet.  He'd lke to combine shipping for the two items.  Do I have to cancel the sold item which will refund his payment, and relist it with the new one for him to repurchase?  Will canceling a purchase count against me in some way and is this the best way to do this?  Thank you.

Since they have already paid for the first purchase the only way to combine was as you mentioned...cancel the first order so that they are refunded and then make one listing for both items.  If you use buyer requested as the reason for cancellation you won't be penalized.
