I really wish ebay would not put "repeat buyer' on a buyer's FIRST item just because they then went ahead and bought something else as well.

The buyer would read "A seller you've bought from before" as they buy the 2nd item. Also annoying during one sitting.

When I have a sale from a repeat buyer, I always perform a maximum 2 year history search to see what they purchased so I can make sure to match it or catch any mistakes. Also, buyers like it when you recognize their continued support and remember them as an individual and not a sales order. This has become habit for me to always do the 2 year history search and it will confirm for you that the repeat buyer is only a repeat because they purchased 2 or more items "today".
Before eBay posted the repeat buyer badge, about 2 years ago, I had made the suggestion to them about 4 years ago that they should alert sellers that a buyer was a repeat maybe by using a different color font on their username. So you can blame me if you like.

Likely just the best that eBay could do/needed to with coding.
I see it.
I find it odd, but as it makes no difference in the overall scheme of life, so be it.
As it causes me no harm, I move along rather unconcerned about it.

I find this annoying too. It actually is the buyer's first purchase from me, they just happened to purchase multiple items. Each time I look it up to see if they purchased from me before when Repeat buyer shows up, sometimes it is true and sometimes it is their first purchase but for multiple items.
Seems to me that Ebay should be able to adjust the code to tell the difference.
