I have not been able to get to the postoffice because of either car issues, or ill health. How can I let buyer know I'' be a couple days late in shipping item?

Go to your sales order and "contact buyer".
Don't tell them about your ill health, it's the oldest excuse in the book. I always say it's due to my work schedule. You want your excuse to not sound like a ploy to get sympathy.

You can send the buyer a message but you're still on the hook with ebay.
Ebay will require you to ship the item on time and will require the item arrive on time.

Go to Orders
Select the Drop Down Menu Next to Get Shipping Label
Scroll down to Contact Buyer and write your message there.

Go to My eBay-> Selling-> Orders-> All Orders
Find the transaction.
There are three dots next to Order Details-> drop down menu
Go to Contact Buyer.
Let the customer know the situation. Offer to cancel if this will be a problem.
If it is a problem for the customer, cancel as Buyer Request.
If you can send FB afterwards (I don't think you can) make it positive "Thank you for pleasant communication." doesn't actually mention the cancellation.
If not send a thank you note with the cancellation notice.

What is your handling time on the item? Do you already have the item packaged and the shipping label created and basically just need to drop it at the post office? If so do as others suggested simply have the PO pickup the item or if you know your mailperson well maybe you could possibly just hand it to them when they deliver you mail, if your mail gets delivered to you.
If you need to go to the PO to actually get the label and ship the item even if the buyer is OK with the delay as inhawaii stated you are still on the hook with eBay and not getting the package shipped on time may result in a ding to your seller performance ratings.
