I shipped the buyer a real Audemars Piguet watch. The watch has been delivered to them now they are claiming the watch is fake. This is a huge scam I am very worried I will lose out on my watch and money. The buyer is requesting to return item but I will not accept a fake watch when I sent them the real thing and lose out. What do I do in this situation??

So you listed it again? Why? What was that going to prove? You still have not explained that. All you did by listing it again was to convince the buyer what you sent him was not a unique watch.
Also why are you so convinced he will send you back a fake watch? All Audemars Piguet watches have unique serial numbers engraved on the back. Surely you can easily prove it by showing eBay the serial number on the back of the watch you sent and the one he sends back. Almost all listings for these watches not only have pictures of the engraved serial numbers on the back, but the original boxes it came in with the certificate of authenticity.
If you don't mind my asking, where did you buy this watch? Many times people buy stuff on eBay and assume since they bought it on eBay, it has to be authentic. Or someone gave you what you thought was a great deal. If you did not buy it in reputable store or a eBay listing with the authenticity guaranteed, you probably were scammed yourself.

First the odds of a person with those type of funds to buy a watch at 18K being a scammer are maybe a tenth of one percent.
Secondly you cannot be out both the watch and money. If they return a different watch, you are out the original watch and the 18K sale really never occurred. Any accountant would tell you you're only out what you paid for the watch originally and the IRS would not allow a loss write off of 36K.
So, accept the return and sees what happens. If you end up being scammed, contact the legal authorities so they can look into your claim. Of course, you would need to provide evidence that you were the rightful owner and of the watches value.
No reason to assume you're being scammed at this point

Buyers can return or steal almost any item from any by filing a fraudulent dispute. This has been true since 2008 when eBay introduced the Money Back Guarantee for buyers.
You can either:
- Accept the dispute and pay return shipping and hope you get your item back
- Decline the dispute and not pay return shipping and not get your item back
Almost every transaction on eBay carries a risk of fraud and loss.

You should have come here first asking what to do.
You would have been told not to list a $18,000 watch here.
Right now, not much you can do but accept the return and see what gets returned.

That watch that sold on December 16 had 22 revisions to the listing between November 26 and December 15. Several of the revisions were to the category, pictures were added and removed, price was changed multiple times, etc. I'm not sure I ever saw this many revisions to a listing!
DISCLAIMER: I'm not a watch expert but I comment on what I see.
Just curious whether anyone else sees the differences I see in this side-by-side comparison.

Return fraud is a big problem. Never sell an item you can't afford to lose.
Just have to wait to see what you get back.

lol the screws into the bezel are backing out and the numbers and lines are way off. If op is lucky the buyer will send back a better fake. too bad no one wants to help this seller figure out how to keep the money!

If a scammer were trying to rip someone off for $18,000 and the buyer wanted a refund, the scammer would come to forum to try to figure out how to keep the $18,000.

I would be worried too about refunding! As a new seller, the funds can be held up to 30 days, longer in some cases (this might be the latter) and any refunds will need to be paid out of a backup funding sources not the money being held. Once the dust has settled and if eBay does not ban the seller in the meantime, the funds would be released to the seller to cover the refund that came out of the seller's pocket.
