Personalization not working on product listing

Im trying to do personalization on golf balls, I turned the option on but nothing shows up? I also talked to ebay and the help desk agent wasn't much help? I also looked at communication preferences as well. I Know it works in this category as I noticed another seller using it and personalization showing up for them. Anyone know what the issue might be?

I see the "Personalize" Item Specific was enabled, but you're right, there's no field to fill in.  Did you put anything in the "Personalization Instructions" field (I think that's the name)?
