I just had a listing completely disappear, not in active, not in ended, even searching the item number didn't bring it up. Turns out a buyer had added it to their cart. I thought adding items to a cart had no affect on listings or stock quantities, only checking out did. Is this normal system behavior? Does it remain invisible for as long as its in a cart? If the buyer doesn't follow through with check out and empties the cart does the item become visible again?

It's gone from your active listings and in someone's cart as soon as someone accepts an offer from you, or you accept their offer - even if they haven't paid for it yet.

Items remain visible until someone buys it.

If it is in someone's cart it should still be visible unless the buyer clicked on 'request total'. Once they do that they have committed to buy it so it is considered sold.

Winner, winner, chicken dinner! The buyer and I were discussing a shipping discount and I directed them to do the 'request total' bit. Though I don't see the logic here, what if they don't like the total? Can they still back out of the transaction at that point?

No matter what, sold, not sold, in a cart, not in a cart, with an offer, not with an offer.... you as the seller would always see your own active listings at https://www.ebay.com/sh/lst/active
and then use one of the filters to find it:
I suppose if eBay pulled it that'd be different, but then again you would have gotten an eBay message about the listing being pulled and why.
But 100% a buyer having it in their cart would not make it disappear for you, the seller in your seller views.

They can, but then it will be an "unpaid item".

They can "back out" in the usual way.....never paying! (or asking to cancel).

As slippinjimmy stated, they can request a cancellation if they don't like the combined shipping discount. Or, they can simply refuse to pay and the seller can cancel because the items were unpaid. The fact that the buyer commits to buying before they know the combined shipping cost can be a problem in some cases. I'm not totally sure what the logic is either but I suppose it guarantees that someone else won't purchase any of the items while the buyer is waiting for a total.
It basically worked like that before the cart too. A buyer had to commit to buy before requesting a total.
