Mailed item with QR code to buyer, they contact me to say it wasn't received and it turned out they had no street address saved in eBay info. So the QR code had no delivery address and it somehow still shipped. It made it to their city and shows "returned to shipper undeliverable" and nothing after that. Yet it has a check that says it was delivered back to me. We have both tried hunting the item but original label from UPS is void and shows nothing, and USPS tracking number it ended up with after transfer is invalid, like neither ever existed. Who is responsible here?

I unfortunately have no answer for you, but a couple observations.
So from what you are saying, the QR code does NOT pull the ship to address from the buyer's order details, but from the buyer's account set up. That I find extremely unusual and not a likely thing to happen.
Have you looked at the buyer's order to see what ship to address they gave you when they paid?

If the QR code had no address for the buyer, how did it get to the buyer's city?
I'd start with contacting USPS. File a Trace on the package.

If the tracking told you it was delivered back to you, where did you see that info since you also say that the tracking number is invalid. It can't be both. And did whatever it is that you saw actually say it was DELIVERED to you or did it say it was being returned to YOU the shipper?

Was the original shipping label from UPS or USPS? The QR Code should match the delivery address provided by the buyer. Was it correct from the onset?

Hi @maes_wings . Typically when you ship with UPS Ground Saver (aka Surepost), UPS will apply address corrections before handing the package off to USPS for final delivery.
Can you confirm when you go here (https://www.ebay.com/sh/ord/?filter=status:ALL_ORDERS ) then click on the order number, the buyer's address is incomplete? Is it correct to assume by "no street address," what you mean is it has a street, but is missing the unit/house number?
If the buyer provided an incomplete address at checkout, then the responsibility here lands with the buyer and you're not required to refund them. If the package is eventually returned to you I suggest refunding them item cost less shipping using the "send refund" option. If the buyer files a non-receipt claim with eBay, eBay will rule in your favor since the scan shows it's undeliverable as addressed.
If the buyer provided a complete address and the issue was with the label generated through the QR code (less likely scenario), come back for more advice.
