I refunded a buyer for an item that was delayed in shipping by the USPS. Well, wouldn't you know about a week later the package is now "Out for Delivery". I was forced to use Managed Payments, so I no longer have a PayPal account. How do I send the buyer an invoice?

I guess you'll have to open another PayPal account. With the buyer's email address, you can send an invoice there.
Why in the world did you do away with your PayPal account? You don't shop here and you use labels from Pending payouts?

Invoices and being worried about the buyer re-paying doesn't really seem to be in ebay's game plan.

This is one very big problem that eBay needs to address sooner rather than later.

Customer is under "no obligation" to pay for item after refund was given. Only thing you can do is appeal to customer's honesty and ask them to mail you a check.
Better to send items by "priority mail" (delivery is very fast & reliable....you won't get stuck with "didn't receive my item" claims.
Also Louis DeJoy is raising costs of first class to 58 ounces and slowing down delivery (up to 5 business days).

Depending on the cost of the item, it might be prudent, as soon as someone files an INR for a late delivery, to request a recall on the package. It isn't free and doesn't always work, but if it does, it can be a whole lot less expensive and painful than getting the laugh from the buyer.

I have been informed by eBay that you are now in possession of the item that was filed and refunded as not received on xx/xx/xx.
eBay have suggested I send my details so you can now repay $ xx.xx as per the original price of purchase.

Once you have been in a transaction with the buyer I don’t think you should have any problem providing your email for Paypal bank deposit details in your message.
As to eBay missing out on fee's for the 'repayment' - tuff luck, the system basically beg's for you to break protocol.
