I want to mark down all of my items 5%. I've tried making this sale a few times the past few weeks, and every time about 200 of my 400 listings don't get included because they 'had a recent change in price'. I haven't changed their prices like this for a few weeks. I tried again today, after putting my store on vacation for a week while I was away, and still the same thing. Why can't I just put all my items on sale 5% and not have items skipped? I check the box to 'Include skipped items if they qualify' and it still skips them. How do I make this work?

How long has it been since you removed your items from 'vacation' mode? Sometimes, it takes awhile (like a few days, sorry) for all items to become 'visible' again, so that might be part of the issue you are having when trying to run this sale.
Also, when you use the markdown are you doing 'sale event + markdown'? If so, then when I run a sale I always select the 'rules' one, I can't remember exactly what it's called, but it's on the right side of the screen when selecting the type of sale. It will pull up to 10,000 listings, whereas the other one pulls up to 500.
I choose that one as it seems to more commonly put most, if not all, of my items on sale, rather than skipping a ton of them. The whole markdown system can be pretty hit or miss in my experience, but that's my best advice. Sorry it's not much

I suggest that you try using order discount from the drop down under create a promotion. On the next page, you should be able to select store wide from the drop down under choose a category and at the bottom of that page there is a choice for no minimum purchase where you can select extra 5% off. On the next page choose create rules and take it from there with the rules that apply to your goal.
