If I sell a pickup only item, am I allowed by eBay policy to accept cash only but no paypal?

Yes, you just need to be very careful how you word your listing to make it clear.

When listing the item, select the "Pay on pickup" checkbox in the payments section, select "No shipping. Local pickup only" in the shipping section and place this text - "Contact me for payment methods to pay on pickup." - in the item description.
You will still need to select PayPal or one of the other payment methods... eBay requires you to select at least one of the approved payment methods.

And if they do take the paypal payment - MAKE sure you get a signed receipt from them accepting the item...otherwise they can state they sent payment but did not receive the item. You have to click on the shipping icon to show it was done. Protect yourself.

In most categories, you are required to offer an electronic payment (paypal or credit card). You are also required to accept any payment that you offer.
However, most buyers understand that cash after inspection is the norm for local pickup, and usually you will have no trouble convincing them to pay that way.
If you accept Best Offers, one of the eBay reps recently stated that you are allowed to state in the listing that you will give preference to Offers that pay by cash.

You can take cash but you are required to include an online payment method as well.
