Customer buys a blade for a PS10.
I ship him a blade for PS10.
After receiving the item he starts a INAD return "wrong item sent".
Said I sent him a PS10 blade instead of a PS30 blade.
I accepted the return and paid for a return label.
I'm waiting for the item to be returned.
Before I got a chance to block him he purchases another blade, this time for a PS30.
Do you guys block customers who return?
Should I have any apprehension about his second purchase?

Buyer simply made a mistake buying the incorrect blade. No block.

I find variations to be confusing when buying. I search for, lets say, a Stihl abc blade. Seller has it listed as a variation, listed with 123, 456, 789, blades. Those show in the title, the abc does not show in the title. But it comes up in the search since it is listed in the item details. I have to search thru the variations to find it. Then I wonder if I will really get what I want, since it is not shown in the title. Or, since it is not in the title, I pass that listing by & buy one that has it in the title, with no variations. Or, I see abc listed with 123, 456 & 789, & I know they are not the same & pass it buy as I suspect the seller doesn't know what they are selling.
Maybe I'm lazy, I don't want to do a few simple clicks to find what I want in a variations menu.

Your listing title looks like one item will fit all 4 different models. I understand it is a variation listing but there are buyers out there that don't realize that you have to pull down the variations and pick one, especially new buyers. I personally don't like variation listings as a buyer. There is always that feeling i get that even though I clicked the right option that I will end up getting the wrong item. If it was me I wouldn't block them and would probably change my listing to individual listings for each variation but that's just me.
As for blocking buyers to return items, it depends on the reason for a return. If it is just the standard "doesn't fit" or pretty much any other reason where the buyer pays for the return I let it go. If however they are abusing the return reason for a free return shipping label or the return is abusing me as a seller (buys 3 of the same item in different sizes and then returns the two that don't fit) they get a insta-block from me. If you want to try things on to see if it fits, go to a B&M retail store and use their fitting rooms. Don't expect me to pay for your shipping to act as your fitting room.

There is no default.
You have to choose one in order to make a purchase.
But if you (and @aeparts1 ) don't like variations listings, I respect that.

I block only when the buyer is rude/nasty or is scamming or not telling the truth (saying its not working when it is)

As long as it's the item you're looking for at the right price from a reputable seller, why would the fact that it's a variations listing make you not want to buy it?
The biggest problem with it is that you see a low price, click on the one you want and the price is actually double in most cases. To me those sellers are trying to trick potential customers into looking. Like the OP's listing. When you see it the listing is for $25 but when you go to buy the one the person wanted it is $45.

YES/NO. I have a Free Return Policy. Having said that. If the buyer is rude, I put them in my block buyers list. If they are nice, I don't bother.

UPDATE 5-18-23
Variations listing for a replacement blade for a PS10 / PS30 / PS40 pruner.
The guy buys a PS10 by mistake, wanted a PS30.
Starts a INAD saying he received the wrong item. I accept the return.
Then he buys a PS30. I ship.
I receive the the PS10 back yesterday. Refund the buyer.
Today he buys a PS40!!!
Any cause for concern?
Does this at least warrant a message to the buyer asking "are you sure?"

If a buyer uses a buyer remorse reason when I'm selling with free returns, I don't block. I advertised the free returns.
When buyers try and bypass my return policies with obvious false claims, that's report and block.
If I ever realize I made a mistake or sent something defective, never would block.
IF they leave negative feedback first then ask about a return, instant block, report.
So far I feel I've been lucky as eBay has removed my negative feedbacks. I've literally never said no to a customer for a return and process a label typically within an hour. Really sad when they give negative feedback when you try your best to give them everything they are asking for. 😞

I just had similar happen. Buyer messaged me 6 times for no real reason, receives item, a headlight. Said, "I've never looked at the back of mine before and yours (which was new in a sealed box that he opened) won't fit. I bought the wrong size." And took photos.
So it's eBay, I said sure I'll send a Refund, nope. I was surprised to find he opened an INAD, which he immediately "won," and eBay said to give him a full refund, keep the item. Really made 0 sense as he bought the wrong item, backed by photos.
Yes, he messaged me more times, do you have this, can I do an exchange, who knows what? I Blocked him. Sorry, but guy was unreasonable. And like any of us, I do not appreciate an INAD when I have photos of the box plus state it's Model XYZ, but you needed Model ABC, except never checked that.

I completely understand the concern, but an email asking them that would likely start problems you aren't likely to want.
