I sold a relatively high value pre-owned clothing item and it went through EBAY authenticity guarantee checks fine as expected since it was in perfect condition. The buyer received the item and now says it does not fit. Starts to manufacture a reason to say the item has been altered etc when it has not and this is easy to check. What can I do? Can they return (with a no returns policy) ? I thought sales like this are final sale otherwise why have an authenticator?

If a buyer wants to return an authenticated item, your return policy as stated in the listing will apply. Authenticated items are covered by eBay Money Back Guarantee, assuming all other eligibility criteria are met.
When returning an item, the buyer will send it back to the authenticators who will check that the item is still in the same condition and has all relevant accessories. Once the item has been re-authenticated, it will be returned to you and the buyer's refund will be processed.
eBay may sometimes issue a refund to your buyer on confirmation the return is on its way. When this happens, we’ll wait until the item is back with you before deducting the amount from you.

There is no "no returns" policy on eBay, whether your item is authenticated or not. The money back guarantee offered by eBay means you have to accept the return, regardless of what your personal or shop policy is. Even with authentication, the buyer can file an INAD. The authenticity guarantee only covers the authenticity of the item. It doesn't cover inaccurate descriptions or missing information. For the sake of your business you may want to offer free returns to protect yourself from buyers who will otherwise use the INAD option for returns. Accepting a remorse return will not affect your account, but an INAD will land you with a defect if eBay has to step in and force you to refund the buyer.

Can they return it with a No Return policy?
It all depends on the reason they select for the reason for the return.

What exact reason does eBay list as the reason for a return? Does not fit is buyers remorse return and can be denied by a seller.

For some categories the "authenticity guarantee" limits the "returns".
Some categories, if the seller does not accept returns, they do not have to accept once the item passes through the eBay process.

So, I notice more than one item authenticated...
One states by eBay no returns...and one states returns OK by the buyer.
So, we don't know which one it is but it usually states on item sold if there are returns or not.

You are selling clothing online and regardless low end or high end clothes, you will inevitably have returns. Besides, the buyers on just about every other platform can return items. You sell some pretty high end clothing and your sales seem good, so at that level, if an item doesn't fit you graciously accept the return. Just the normal cost of business. You may have to change your margins if returns become a problem.
