Calculated shipping charging too much

I could really use some help. I sell one basic type of item with generally a consistent weight and size. I have all all my listings set to calculated shipping and specify a handling cost in the listing. I list each item as weighing 4oz and ship via priority mail in a 7x7x6 inch priority mail box. I offer combined shipping. I specify a handling cost of $1.25 per each additional item. I have the box checked to apply my calculated shipping rule. Every item is listed with identical shipping.  Some of my customers (not all) are being overcharged and I cant figure out why. 

 

For example my last order. 

I ship from Portland Oregon

My customer lives in Illinois

I ship via Priority mail in small priority box

She ordered 4 plants which I specified a weight of 4oz each so that should  weigh exactly 1lb 

I charge a handling cost of $1.25 per plant on top of the calculated shipping.

Retail shipping is $9.10

Plus add the 4x$1.25= $5

She should only be getting charged $14.10 for shipping 

 

But she was charged 23.19!?!?!

 

I really don't understand why this is happening to some of my customers. 

 

Can somebody take a look at my listings to see if there the shipping is going wrong?

Calculated shipping charging too much

Nevermind. Found a checkbox for apply my calculated shipping rule on one item that wasn't checked. Only the orders with that particular item were being overcharged. Sorry.
