They want to return the item. I told them to send a return request under the account they made the purchase but they say it was a guest account and can't access it. The only reason i'm not issuing a refund or sending a return label is because i'm not 100% sure this person is my buyer. How would you handle the situation?

Send this to your buyer. Getting help if you bought as a guest. Also the purchase would be in the email account they used to make a purchase as a guest tell the buyer to look there as well.
I wouldn’t give any refund until a return request is opened and the item returned.
