Let's just say his ID is 000 and he is in San Jose. I blocked him for making lowball offers and then repeatedly sending messages that my items have not sold for months and asking what would be a price for 4 books. I thought that was too much a risk for someone who offered $5 for a $50 book. He has 100% feedback, mostly from the big book dealers. I suspect he buys these to resell (which in itself is not a problem)
Am I being paranoid?

What is your fear exactly? That he intends to receive the books, claim SNAD, make you pay for return shipping and return some garbage to you, thus cheating you out of the books he thinks he should have been able to get at pennies on the dollar?

Also I am not sure how you would be able to prove it, but if you can somehow prove it or want ebay to look into it, it may be worth giving them a call.
If it is indeed the same person and he's gone around your block that is a very serious thing and sanctions could be brought upon his buying problems for attempting to circumvent your block.

What is your fear exactly? That he intends to receive the books, claim SNAD, make you pay for return shipping and return some garbage to you, thus cheating you out of the books he thinks he should have been able to get at pennies on the dollar?

Yes, I guess so. I just never had someone continually harass me after turning down an offer.
If your gut and the evidence you have on hand tell you this is the same person I would try to call ebay ASAP and explain the situation and see if there's some way they can figure it if it is indeed the same person trying to get around your block.
If so, I would hope that some kind of suspension is handed to this person not only for their harassment but for attempting to circumvent your block.

Nope. Ebay no longer cares when a buyer circumvents your block as long as he does not do it with the intention of causing harm to your business.

And how exactly would ebay determine whether or not the buyer intends to cause harm to your business?
If you block someone and have good reason to (as the OP does here), then you should not be forced to sell to that abusive person.

This happened to me. I called ebay before I shipped the item and made sure I got a Convo number (or whatever it's called). I had the CS read the emails and she told me that if I received a SNAD or negative feedback, ebay would protect me. Fortunately they bought the item, did not leave feedback, and I never heard from them again. I'm glad because I don't know if ebay really would have protected me.

OK, update:
I just got off the phone with customer service. I had to explain my situation about 3 or 4 times. I think the CS rep was not a native English speaker. He claims that I have the right to cancel the order if I suspect this is the original blocked buyer but I was not too confident with his answer.
What the heck, I might just mail it out on Monday and take my chances.

Well whatever you decide, best of luck.
If you do decide to cancel it, even thought it's not actually proper, I would say this would be a case where it would be excusable to lie and say "at buyers request" so you don't get a defect.
I doubt he will call ebay and try to report you, but you have recourse if he does.
I wouldn't do recommend doing this at any other time since you could be subject to a defect if the buyer reported you for using that reason improperly.

Last year, Ebay reversed this policy and said that it is ok for the buyer to do this if he really wants your item. It has been said that IF you have a problem post sale, then you can call CS and they will help you with it.
However, since then, I believe Trinton reiterated that it is a serious violation and Ebay will protect you. Possibly the meaning is the same ~ they want to know and if you have a problem post sale, CS will protect you and help you with it.

I know it used to be a serious violation to circumvent a block, but then last year, it was changed to it's ok if the buyer really wants it. So is it now back to being a serious violation, but still ok since CS will protect you? Please advise, thank you very much!

Wrong info - never use " at buyer's request " unless the buyer specifically tells you they don't want the item in an Ebay message. Too often sellers use this to cancel and the buyer calls and complains. You not only get a defect but they'll be watching you.
It's been said beore that even tho the buyer is blocked and creates another ID to circumvent that block - you should mail the package. Ebay supposedly will help a seller if the buyer tries anything malicious.

I think a lot of people put out low ball offers and some are more aggressive about getting a seller to lower the price because they do this basically for a living and they may have numerous low ball offers out here. If they go ahead and change an ID after they have been blocked they might still be an OK buyer just trying to get the lowest prices on stuff, and when they cannot they just buy it. It's technically not allowed to circumvent a block here and you can see if that is what has happened but, now that the item has sold, you are in a sticky situation regarding a defect if you cancel the sale. your call as far as going through the calls to Ebay to see if you can get the sale cancelled based on the block but, what a hassle.
