I started back selling again and sold an item late Thursday night. The terms are I ship within 5 business days, eBay says item needs to be shipped before the 16th. The order was planning on shipping out first thing Monday morning, well the buyer filed an item not described doesn't match the description title Friday night! eBay has the money on hold until I receive the return and refund the money two days after the return is shipped!
Is it wise to go ahead and refund this order? If I do, will I get strikes against me?
I don't understand how eBay allows a return process to be started when the order has not shipped!
Scared if I this item, the buyer will definitely send some different back!

the buyer likely changed their mind or made a mistake. You don't have to assume the worst in this case. Reply to the request with "I have not sent out your item yet. Would you like to cancel the order?"

First thing I would do is message the buyer and ask them if they are confusing me with another seller.
Message something like:
I'm sorry that you are unhappy with "said item" but I did not send you "said item".
The item you purchased from me is sitting here on my packing table waiting for the shipping label to be attached. It was scheduled to go out in the mail on XX/XX/XXXX .
Did you maybe confuse me with another seller ?
Only then would I try to answer to the case opened.

The buyer is already doing a return request. I don't think ebay would let them do an order cancelation too?

The fastest & easiest way to resolve this is to refund the buyer immediately, block them and NOT communicate with them at all. Yes, maybe they made an error and confused you with another seller, but you will be better off if they don't have a chance to make even more errors that could end up even worse.

Well, I would contact buyer and tell them I was cancelling their order per their request. Cancel and refund money. If it's not been shipped, then just cancel per customer.

Sorry -- I am confused. "The order was planning on shipping out first thing Monday morning . . ." Did you mean today, Monday, the 14th of June? Did you ship it today? If that's the case, how could the buyer have opened an item not as described case when he cannot possibly have received the item yet?
Why not cancel, citing buyer requested it?

I think the question is can they cancel with a case already opened?
I would not mail and call CS and see what they say........perhaps they can cancel it.......

Just refund the purchase price and get on with your life. You are making things more complicated that they are.
