The header ProductName is part of the "Selling Manager Pro" CSV files and not the File Exchange template. I am not sure which your trying to use but if it is File Exchange template your trying to find then go back into the File Exchange Center and click on "File Exchange" which is below the words "Select a Template" and then choose the category template.
