Send it back to the return address on the label, if there isn't a return address, keep the item, give it away or throw it away.

First, make sure that it's not sent from a seller from whom you have ordered.
After that, hold on to it for awhile.
I, personally, wouldn't bother returning something until/unless I had been reimbursed first by the sender for the return shipping cost.

If it's not from a seller you ordered from, and nobody in your house ordered it, you can just write "RETURN TO SENDER" in big bold letters across the label, put it back in your mailbox or take it to your post office (the sooner, the better), and it'll be returned for free. My favorite method is to drop it in one of those big blue mailboxes so I don't have to deal with people.
This also works great with junk mail, FYI.

Actually, stuff the junk mail back in the prepaid return envelope and mail those back so they pay that postage.
Your name and address are on the junk mail* stuffed inside, so they know to take you off of the list, working like Return To Sender does to take you off of the list.
*torn up, so they can't say you accepted the offer
