Go to your ebay purchase history and find the item. Select More Actions,  then select Return Item. Follow the prompts.  

Come back here with any questions that arise.

If you're asking how to open a Return claim, you'll find the item in your Purchase History and press the big, blue 'Return this item' button to the right. Cite the reason for your Return, Confirm and Submit. Good luck.
