I guess I never previously had any returns from Guamanians up until now, could that be true?
Guamanian buyer filed a false SNAD and I didn't know the buyer never got an ebay return label until recently. The return status was stuck at "waiting for buyer to ship item back" just like any other. But 3 days left on that time window, the buyer contacts ebay and the return gets pushed up into a case saying I (the seller) needs to take action and resolve it. An email followed with the notification, the buyer called ebay and said they never got a return label. So I have to send them one myself and resolve in 3 days or I'll get funds pulled and a defect.
That's when I found out the buyer was in Guam. Aside from other US territories, Guam requires a customs form. And that's why I believe ebay wasn't able to issue the return label as well as me not being able to generate one from Pirateship too. I spoke to CS about this and he agreed that Guam is a headache about this, he mentioned Saipan is like that too. He said the only way was to pay them through paypal for the cost of shipping it back. I've been burned too many times to agree to that (buyer packing way too big or heavy so I'd be saving money by not getting the item back, or the buyer insisting on taking the money first because of lack of trust then they keep the money and never return the item back). In the end the business choice to cut my losses at a minimum was to just refund the buyer.
This buyer had a big buying history and a buyer feedback score in the thousands. You would think that someone in Guam who bought that much from the states would have had at least a few previous return issues to already know that he can't get a provided return label. Quite the loophole for Guam and Saipan buyers.

Yeah, it's a problem with US territories and not just for return labels. A lot of sellers have reported that eBay isn't generating the customs form for outbound shipments so those packages are being returned to sellers.
I don't know if you can generate a return label through pirateship when customs is involved, but I would think you can. At the very least if you can't do a formal return label, you can generate an outright prepaid label with buyer's info in ship-from field and the customs form should populate.

After the typhoon yesterday there's not much that can be done one way or the other. I believe there is not a lot of electricity on the island. Nothing can get return to you and nothing you can do at this point. Probably no contact with the buyer as well. Out going mail I am sure it is at a standstill. Best to just refund now so there is no defect.

Should the seller refund again @12345jamesstamps (is there a way to do that on eBay) after they already refunded?

I have refunded a buyer for shipping discount...and you can refund them again for item not received.
